These are follow up visit types combined together for a discounted rate, specifically for BCBS and Anthem clients.

Package 2

Three (3) follow up visits at our office
$450 ($525 value)

Package 3

One (1) follow up visit at your home,
Two (2) follow up visits at our office
$475 ($575 value)
